The plan will reimburse you if you have to travel 100 miles or more, each way, to see a provider.
How it works:
Use your or someone else’s vehicle to get to your doctor visit at least 100 miles from home. Once we receive the necessary information, we’ll send you the standard Internal Revenue Service (IRS) reimbursement rate per mile.
To qualify for reimbursement:
You must have a necessary medical or dialysis treatment that is covered by your plan
You travel at least 100 miles from home for your visit
You send us documentation about your trip
To get reimbursed, you’ll send us:
Documentation showing how far you traveled for covered medical services
- Copies of bills
- Paid receipts
- A list of expenses for which you’re asking us to reimburse you
Download and complete the Travel reimbursement form. Then send it, with the documents above, to the address on the back of your ID card.
